Painful corns can become a serious problem when constant pressure is involved. This elderly patient presented with a corn on the PIP joint of the second toe that had begun to ulcerate. The underlying cause was tight footwear compressing the toes from above, which increases pressure on the joint. Over time, the skin responds by thickening and hardening. That hardened skin creates even more pressure, forming a corn — and if the pressure continues, the surrounding tissue can break down and start to ulcerate. Today’s treatment focused on carefully removing the thickened skin and corn to relieve pressure and reduce pain. Immediate relief is usually felt once that hard tissue is removed. In this case, the patient prefers ongoing conservative care rather than surgery or footwear changes, so we’ll manage the problem with routine treatments every 6–8 weeks to keep the area comfortable and prevent further breakdown.
